I have added rbl lists through E.Manager -> mail server -> setting -> rblhosts, and I have seen mail being blocked.

but I've also got a complaint from a client saying while sending mail, he's got the following response "'451 open proxy -- 1076220004". does rbl setting block both incoming and outgoing mail (sent by clients through qmail smtp)?

If your client is using your mail server as their out going mail server, it is possible you are listed; put your mail server IP in http://www.dnsstuff.com/ (spam database area) and check.

If your client is using their ISP as their outgoing mail server, it is possible your client's ISP is listed.

Use the NS lookup on http://www.dnsstuff.com/ to find the IP addresses associated with the outgoing mail server name, and then check that in the spam database.
